We uphold the same ethical standards consistent with national laws for client privacy and those laws enforced by our professional board, the California Board of Behavioral Sciences. Each client will receive and have a chance to review and provide their full verbal and written consent prior to receiving our professional services. The informed consent will consist of information provided regarding our policies and procedures consistent with the Health Insurance Portability and Accountability Act as well as potential benefits, risks and expected outcomes of the therapeutic approach used in which each client agrees to take part. The client intake form aids the first clinical interview/assessment. It is a standard tool used at the beginning of professional counseling services to help your therapist obtain essential information about symptoms and complaints and will be used to form diagnostic impressions and guide subsequent clinical treatment.

It is important to us that you understand your vital role in the client-therapist realtionship. Please don’t hesitate to ask your therapist any questions we may be able to answer as we work to develop a counseling plan together

If you are able to prior to your first appointment, please take some time to review the forms and information about our services provided here.  We look forward to working with you!
